Original valid passport at least a 6 month validity from the date of travel (Old passport whenever available).
1 Visa form filled all details and signed by Applicant as per passport.Hand written visa application forms are not accepted.
2 Recent Coloured Photograph (Fresh ones i.e. NOT used earlier for any visa) should be: –
- 35mm x 45mm in dimension
- On matte finish paper
- With White background
- With 80% face coverage, non-smiling [without sunglasses, a hat/cap or other head covering, unless the applicant wears such items because of their religious belief or ethnic background
- Not previously used in a visa

Covering Letter Addressed to Consulate General of the People`s Republic of China, Kolkata.
Invitation Letter Addressed to Consulate General of the People`s Republic of China, Kolkata. Invitation letter format attached in the Forms section.
Original Work Permit – issued by the Ministry of Human Resources and Social Security of the PRC.
Registration Certificate of Resident Representative Offices of enterprises of foreign countries (regions) – issued by Chinese authorities of industrial and commercial administration.

Medical Requirement

Physical Examination Record from any Indian Government/State-run Hospital. The photo on the Record should also be stamped by the Hospital (Valid for six months from the issued date).
